How to apply for a certificate

You can obtain a certificate by providing:

  • the required information;
  • reason for the request;
  • relationship to the person named on the certificate;
  • type of certificate requested;
  • a cheque or money order made payable to Service New Brunswick in the appropriate amount or by providing your VISA, MASTERCARD or AMERICAN EXPRESS number with expiration date;
  • full return mailing address and daytime telephone number.

Certificate applications can be made online, in person or forwarded by mail:

You can now apply for a certificate online.

You may only use the online service to:

  • Apply for a birth certificate for yourself or your minor child
  • Apply for your marriage certificate
  • Apply for a death certificate

In person

At one of our Service New Brunswick service centers.

Locations by Community - SNB

In person applications for a certificate will require the applicant to provide the following:

  • The written consent of the person listed on a birth or marriage certificate when the applicant is not a person named on the certificate.
  • The written consent of a parent when applying for the certificate of a minor child for which they are not a parent.
  • Proof of death when requesting a certificate where person(s) named on the certificate are deceased.

Birth Certificate
You may complete and print the Birth Certificate Application form (pdf) and submit by mail with payment.

Marriage Certificate
You may complete and print the Marriage Certificate Application form (pdf) and submit by mail with payment.

Death Certificate
You may complete and print the Death Certificate Application form (pdf) and submit by mail with payment.

Note:
If a certificate is required for a person who was not born, married or deceased in New Brunswick, but who was born, married or deceased in a different Canadian jurisdiction, please refer to the list of contacts for out-of-province Vital Statistics agencies.
If a certificate is required for a person born, married or deceased outside Canada, please contact your local Vital Statistics Office for assistance.
