You can obtain a certificate by providing:
- the required information;
- reason for the request;
- relationship to the person named on the certificate;
- type of certificate requested;
- a cheque or money order made payable to Service New Brunswick in the appropriate amount or by providing your VISA, MASTERCARD or AMERICAN EXPRESS number with expiration date;
- full return mailing address and daytime telephone number.

You may only use the online service to:
- Apply for a birth certificate for yourself or your minor child
- Apply for your marriage certificate
- Apply for a death certificate

In person applications for a certificate will require the applicant to provide the following:
- The written consent of the person listed on a birth or marriage certificate when the applicant is not a person named on the certificate.
- The written consent of a parent when applying for the certificate of a minor child for which they are not a parent.
- Proof of death when requesting a certificate where person(s) named on the certificate are deceased.
